The way i see it, Caska's mental state is too strongly tied to Guts' character development for her to just be "All better" all of a sudden, it's going to be a long time before she's cured, if ever. Think about it, what will Guts do if Caska is magically healed? Go off hunting the Godhand? ...

Miura might take long ass breaks, but you can damn well see he isn't just lazing around, the art just keeps getting better.

I doubt she's going to be an enemy, "Isma" essentially means "Safe", and considering how different her name is from others i doubt it's a coincidence. More likely i expect she wasn't corrupted by the sea god because of her heritage, and she's going to tag along with Guts to Elfhe...
